Go to Evaluation Form Templates
From your dashboard, navigate to:
Settings → Templates → Evaluation Forms
Here, you’ll see all existing templates your team has created. Each one can be reused across jobs, departments, or interview stages to standardize candidate evaluations.
💡 Templates keep interview feedback organized and make it easier to compare candidates across the same criteria.
Create a New form
Click on "Set a New form" This opens the Evaluation Form Builder.
Start by naming your template clearly — for example:
“HR Interview Evaluation”
“Technical Interview Feedback”
“Final Round Scoring Sheet”
📌 Clear naming helps your team quickly pick the right form during interviews.
Add Questions to the Evaluation Form
You can choose from the following types:
Rating Scale – e.g., 1 to 5 stars for “Problem-Solving Skills”
Short Answer – for brief comments
Long Answer – for detailed notes or feedback
Yes/No – for simple evaluation criteria
Multiple Choice – for selecting specific categories
📌 You can mix question types to make your evaluations both structured and flexible.
Add an Overall Recommendation Field (Optional)
At the end of your form, you can include a summary question field such as:
✅ Recommend for next stage
⚪ Maybe / On Hold
❌ Not a fit
This helps decision-makers review at a glance whether a candidate should advance or not.
Save Your Template
Once your sections and questions are ready, click Save Template.
Your form will now appear in the list of evaluation templates under Settings → Templates → Evaluation Forms.
Use Your Template During Interviews
When it’s interview time, you can use the evaluation template directly inside the candidate’s profile:
Go to the candidate’s profile.
Click the Evaluation tab.
Hit + New Evaluation.
Choose the appropriate Evaluation Form from your saved templates.
Fill in your scores or comments during the interview.
💡 Every interviewer or stakeholder assigned to that stage can open the same candidate profile, fill in their evaluation form, and submit feedback in real-time.
Once you submit your form:
The evaluation is saved under the Evaluation tab.
Each submission shows the form name, stage, date, and who submitted it.
All evaluations remain visible to authorized team members for that job.
This means you can instantly compare feedback from different interviewers, all organized under the same candidate and stage.
