Create a Questionnaire Template

In this article, you’ll learn how to create Questionnaire Templates in Recruitera — so you can easily collect structured information or pre-screening answers from candidates at any stage of the hiring process.

Go to Questionnaire Templates

From your dashboard, navigate to:
Settings → Templates → Questionnaires

Here, you’ll see a list of all your saved questionnaires. Each one can be attached to workflow stages or sent to candidates automatically.

💡 Questionnaires help you collect key data before interviews — like work availability, expected salary, or language proficiency — without manual emails or calls.


Create a New form

Click on "Set a New form" This opens the Evaluation Form Builder.

Give your questionnaire a clear, descriptive name, such as:

  • “Pre-Interview Screening”

  • “Technical Skills Assessment”

  • “Cultural Fit Questionnaire”

Then, choose the Questionnaire Form Category for easier usability e.g. Survey'

📌 Clear names make it easier to reuse questionnaires across multiple jobs or departments.


Add Questions to your Questionnaire Form

Click + Add Question to start building your questionnaire.

Choose from these available question types:

  • Short Answer – For quick text responses (e.g., “What’s your current notice period?”)

  • Long Answer – For open-ended questions (e.g., “Tell us about your most successful project.”)

  • Multiple Choice – For predefined options (e.g., “Which shift are you available for?”)

  • Yes/No – For simple binary responses (e.g., “Are you willing to relocate?”)

  • Salary Range – For expected compensation inputs

💡 Use short and clear questions that help you qualify candidates faster without overwhelming them.


Set Question Preferences

For each question, you can customize how candidates respond:

  • Toggle Required ON if candidates must answer before submission.

  • Reorder questions by dragging and dropping them for better flow.

  • Remove a question using the 🗑 icon if you no longer need it.

📌 Keep only essential questions — shorter questionnaires improve completion rates!


Save Your Questionnaire Template

Once all your questions are added, click Save Template.


You’ll see it listed under Settings → Templates → Questionnaires. Where you can edit or duplicate it anytime to fit different hiring needs.


Use Your Questionnaire Template in Workflows

You can attach your saved questionnaire to any workflow stage.

To do this:

  1. Go to Jobs → Edit → Workflow tab.

  2. Select the stage you want (e.g., Phone Screen or Technical Assessment).

  3. Click + Add Automated Action → Questionnaire.

  4. Choose your saved template and click Save.

Now, whenever a candidate reaches that stage, Recruitera automatically sends them the questionnaire.
