You can update an evaluation form's questions, scorecards, and sections, rename it, or delete one you no longer need. This requires a role with permission to manage templates & workflows (typically Admins).
Before you start
Required role: a role with the Manage templates & workflows permission (typically Admins)
Where to find it: Settings → Templates → Evaluation Forms
How to edit an evaluation form
Open Evaluation Forms and select the form from the list.
Add, edit, reorder, or remove questions, statements, and sections, and adjust Required.
Rename it with the pencil next to the title, then save.
How to delete an evaluation form
Select the form and click the delete (trash) icon.
Confirm the deletion.
Frequently asked questions
Will editing a form change assessments already completed?
No. Completed evaluations keep the questions they were filled in with. Edits apply to future assessments that use the template.
