Recruitera's evaluation form templates let Admins create structured scoring sheets that hiring managers and interviewers use to assess candidates consistently. Instead of collecting informal feedback in comments, evaluation forms ensure every interviewer scores the same criteria — producing comparable, reportable data across your entire pipeline. This is an Admin-only action in Recruitera.

How to create an evaluation form in Recruitera

  1. Go to Settings → Templates → Evaluation Forms.

  2. Click + New Evaluation Form.

  3. Enter a Form Name (e.g. "Technical Interview Scorecard", "Competency Assessment", "Final Round Evaluation").

  4. Add your evaluation criteria. Supported question types:

    • Rating Scale — score a criterion on a numeric scale (e.g. 1–5 or 1–10)

    • Short Answer — interviewer writes a brief observation

    • Long Answer — interviewer writes a detailed assessment

    • Yes / No — pass/fail criterion

    • Multiple Choice — select from predefined options

  5. Set the weight of each criterion if you want Recruitera to calculate a weighted total score automatically.

  6. Mark criteria as Required so interviewers cannot submit without completing them.

  7. Click Save Form.

How evaluation forms are used in Recruitera

Evaluation forms can be assigned to interviewers from a candidate's profile using the Assign Evaluation feature, or added as an automated action in a pipeline stage. When an interviewer completes the form, their scores and comments are saved in the candidate profile under the Evaluation tab — visible to Admins and Recruiters for comparison across all evaluators.

In Recruitera, evaluation forms are completed by interviewers and hiring managers to assess candidates internally — they are not seen by candidates. Questionnaires are sent to candidates as part of the application or pipeline flow, asking them to answer questions directly. Evaluation forms appear in the candidate's Evaluation tab visible to the hiring team. Questionnaire responses appear in the candidate's Questionnaires tab and are submitted by the candidate themselves.
