Create and Manage Evaluation Templates

In this article, you'll learn how to build your evaluation templates — the forms your team will use to assess candidates. You can create multiple templates for different roles or assessment types, each with its own categories and questions.

Step 1: Go to Evaluation Forms

From your dashboard, go to Templates in the left sidebar, then click on Evaluation Forms.


Step 2: Create a New Template

Click on + New Evaluation Template in the top right corner.

A pop-up will appear asking you to fill in three fields:

  • Template Name — Give your template a clear name (e.g. "Full Candidate Evaluation" or "Technical Interview Form")

  • Form Description — A short description of what this template is used for

  • Template Category — Choose the category this template falls under

📌 If no categories exist yet, click the Template Category dropdown and select + Add New Category to create one first.

Once all fields are filled, click Create.


Step 3: Add Sections (Categories) to Your Template

After creating the template, you'll land on the template builder. Here you'll see a Category Name field — this is where you start building the sections of your evaluation form.

  1. Type the name of your first category (e.g. "Communication Skills")

  2. Click Save & Add Question to lock in the category name and start adding questions to it

  3. Repeat for each additional category by clicking + Add New Category on the bottom right

📌 Think of categories as sections inside your form. Each one groups a set of related questions together and gets its own rating out of 5 when the evaluator fills it in.


Step 4: Add Questions to Each Category

Once you save a category, the question builder appears underneath it.

For each question:

  1. Type your question in the Question Text field

  2. Choose the question type from the dropdown — options include Short Answer, Long Answer, Single Choice, Multiple Choice, Yes/No, and Date

  3. Toggle Required ON if the evaluator must answer this question before submitting

  4. Click + Add New Question to add more questions to the same category

📌 For Yes/No, Single Choice, and Multiple Choice questions, you'll also see a Mark as Disqualify option next to each answer. Use this if a specific answer should flag the candidate as disqualified.


Step 5: Save Your Category

Once you've added all questions to a category, click Save at the bottom of that category block before moving on to the next one.

💡 You can drag and drop questions within a category to reorder them using the handle icon on the left of each question.


Step 6: Edit the Template Later

You can always come back to any template and edit it. From the Evaluation Forms page, click on the template name to reopen the builder. Use the pencil icon next to the template title to edit the name or description.
